These Terms and Conditions govern your use of the services provided by Tay Sports Therapy. By booking an appointment or engaging with our services, you agree to comply with these terms.
1. Definitions
• Patient: Any individual or entity engaging the services of Tay Sports Therapy.
• Tay Sports Therapy: Refers to the business, staff, and representatives delivering sports therapy and related services.
• Services: Refers to all services offered, including, sports and soft tissue therapy, injury assessment, rehabilitation programs, and coaching.
2. Scope of Services
• Tay Sports Therapy provides sports therapy, injury prevention, rehabilitation, and related services. Services may include consultations, physical assessments, treatment sessions, and ongoing support.
3. Booking and Appointments
• All appointments must be scheduled in advance and are subject to availability.
• Appointment confirmation will be sent via email or SMS once the booking is finalised.
• Please arrive on time. Late arrivals may result in a shortened session without a reduction in fee.
4. Cancellation and Rescheduling Policy
• Cancellation: Patients may cancel or reschedule appointments up to 24 hours before the scheduled time without penalty.
• Late Cancellation: Cancellations within 24 hours of the appointment time may incur a cancellation fee of up to 50% of the service cost.
• No Show: Failure to attend without notification may result in a charge for the full session fee.
5. Payment Terms
• Payment is due at the time of service unless other arrangements are agreed upon in advance.
• Accepted payment methods include cash, card and bank transfer.
• Packages or prepaid sessions must be used within the validity period outlined at purchase. Refunds on unused packages are not typically provided.
6. Health and Medical Information
• Patients must provide complete and accurate health information. Tay Sports Therapy relies on this information to deliver safe and effective services.
• Any changes to the patient’s medical condition must be communicated prior to each session.
• Tay Sports Therapy reserves the right to refuse treatment if it is deemed unsafe or if the necessary medical information is not provided.
7. Liability Waiver
• By engaging in Tay Sports Therapy services, patients acknowledge that physical activities may carry inherent risks.
• Tay Sports Therapy and its staff are not liable for any injuries or health issues arising from participation in activities, except where gross negligence or misconduct has occurred.
• Patients assume full responsibility for consulting with their healthcare provider regarding participation in therapy sessions if they have a medical condition or concern.
8. Privacy and Confidentiality
• All patient information is treated as confidential and will only be used for the purpose of providing services.
• Tay Sports Therapy adheres to data protection laws and does not share patient information with third parties without prior consent, except where required by law.
9. Minors
• Patients under 18 must be accompanied by a parent or guardian during consultations and sessions, who must also consent to these Terms and Conditions.
10. Code of Conduct
• Patients are expected to behave respectfully toward Tay Sports Therapy staff and fellow clients.
• Any inappropriate behavior may result in termination of services.
11. Intellectual Property
• All content provided by Tay Sports Therapy, including written materials, exercise plans, and training videos, is protected by copyright.
• Patients may not reproduce, share, or distribute Tay Sports Therapy materials without prior written consent.
12. Changes to Terms and Conditions
• Tay Sports Therapy reserves the right to update or modify these Terms and Conditions at any time. Notice of changes will be provided on our website or through direct communication.
13. Governing Law
• These Terms and Conditions are governed by and construed in accordance with the laws of Scotland, UK. Any disputes will be subject to the exclusive jurisdiction of the courts of Scotland, UK.
